Transitional Care
Guardian Angels Care Center’s Transitional Care Unit helps bridge the gap between hospitalization and returning home. Our 24-bed unit offers a unique residential-style environment including private care suites that include private bath with shower, telephone, flat-screen television (with cable included), refrigerator, microwave, and wireless connection for your computer. In addition to all of these amenities in your room, the Transitional Care Unit offers:
- Dedicated dining room with waterfall wall
- Living room with fireplace
- Two sunrooms
- Whirlpool spa
- Adjacent therapy services
Our Transitional Care Unit is designed to provide short-term rehabilitative stays averaging 30 days. We provide services to clients with:
- Orthopedic rehabilitation
- Cardiac recovery
- Respiratory illness
- Gastrointestinal disease
- Neurological care
- Post-surgical recovery
- Complex medical management
- Reconditioning, strengthening
All suites in the Transitional Care Unit are Medicare certified. In addition, Guardian Angels contracts with a variety of insurance providers.
